myopic astigmatism up to -6.00 DMRSE, with cylinder between 0.00 and -3.00 D in
patients 21 years of age or older, with an Optical Zone of 4.0 to 9.0 mm and treatment
of 4.5 and 9.5 with an Optical Zone of 4.0 to 9.0 mm and a Treatment Zone of 4.5 to
